Partners In Health Sierra Leone LLC (PIH) began supporting the government of Sierra Leone in response to the Ebola epidemic in October 2014. PIH SL’s work is focused in Kono District, which has one district hospital, Koidu Government Hospital (KGH) that is the Comprehensive Emergency Obstetric and Newborn Care (CEmONC) facility, as well as 94 peripheral health units (PHUs). These facilities serve a total population of 505,767. Among the PHUs, 5 facilities have been designated to serve as Basic Emergency Obstetric and Newborn Care (BEmONC) facilities. PIH is currently providing support at KGH with CEmONC services and plans to expand its support to BEmONC facilities in Kono District. PIH also runs a private primary care clinic and maternity centre, Wellbody Clinic (WBC), which was founded by Wellbody Alliance in 2007. WBC provides reproductive, maternal, and newborn health services, including a labour and delivery centre. PIH SL plans to build a strong MNH Program through strengthening CEmONC services at KGH, BEmONC and primary care services including antenatal care, postnatal care, family planning, HIV/TB management at the PHUs and WBC, while also expanding to community level programs building on our previous successes in HIV/TB and Ebola survivor community health worker (CHW) initiatives. PIH SL plans to develop a model health system in Kono District in partnership with the MOHS and District Health Management Team (DHMT), placing major emphasis on the district referral hospital, Koidu Government Hospital (KGH) and the Wellbody Primary Care Clinic & Maternity Centre.

PIH SL is ambitiously setting out to transform Koidu Government Hospital into the leading Maternal & Newborn Health hospital in West Africa, serving as a Centre of Excellence in both the delivery of quality clinical care and as a health care teaching institution for the region. Plans to launch this Maternity Centre of Excellence (MCoE) are in progress.

Position Overview:
PIH is seeking an experienced medical officer (MO) to work in clinical programs at PIH-supported healthcare facilities in Sierra Leone. The MO will be based at the Koidu Government Hospital (KGH) in Kono District:

–       He/she will work closely with the Sierra Leone MoHS and PIH staff on the development and implementation of work plans and initiatives to improve the quality of maternal and newborn care.

–       He/she will provide support to the Reproductive, Maternal, and Newborn Health Clinical Program Manager, Chief Medical Officer and other PIH leadership and advance PIH’s core mission of health systems strengthening in Sierra Leone.

·         Works in collaboration with the PIH Obgyn clinicians, RMNCH Lead and MOHS to provide. comprehensive care for maternity and Gynae patients at KGH, WBA and PHU’s supported by PIH.

·         Supports and Mentor’s CHO’s/students assigned in Maternity/Obgyn and SCBU Wards.

·         Carry out daily rounds on Maternity and Gynae wards at KGH with clinicians and nursing staff.

·         Provide direct clinical care and manage Obs/Gyn cases according to national and international clinical guidelines, including providing surgical coverage for obstetric cases.

·         On call duties to in collaboration with other clinicians to ensure that the maternity unit has 24 hour coverage.

·         Oversee ANC, high risk Obs/Gyn, family planning, AYFS and out-patient clinics

·         Clinical oversight of the GAC and MCoE programs at KGH.

·         Perform clinical teaching, mentorship and capacity building of clinicians through participation in district hospitals, outpatient clinics, community health clinics, case review, and other activities.

Clinical Supervision & support

·         Maternity/Obgyn wards in KGH and Wellbody,

·         Direct supervision for CHOs placed in the maternity unit.

·         Provide supervision to social workers, in conjunction with nursing department.

·         Work in collaboration monthly and as needed with Medical Director; RMNCH Lead; Director of Nursing and Director of Operations on guiding KGH strategy.

·         Visitors and other meetings/events at KGH, as needed from senior management

·         Referral team – supporting Director of Nursing and Social Work as needed for maternity patients requiring referral for diagnostic or specialty services

Administration

·         Attend Clinical Ops Meetings at KGH

·         Attend and compile reports for MDSR meetings in conjunction with the team

·         Attend KGH/maternity morning report

·         Work closely with the PIH SL Medical Director and Kono Leadership team on strategic planning and budgeting for maternity/obgyn work.

·         Work with the leadership team to contribute to grant proposals and reports to current and potential donors

Education

·         Assist with and collaborate on supervision, curriculum building and implementation/scheduling of nursing and CHOs

·         Continue ongoing education curriculum for CHOs

·         Provide bedside training for all cadres of staff

Additional skills/ knowledge/experience preferred

·         Experience in treating malaria and other tropical diseases

·         Strengthen the clinical skills of Midwives, Nurses, CHO’s, students at Koidu Government Hospital and other project sites in the delivery of normal maternal care, BEMONC, CEMONC, RMNCH best practices, compassionate and respectful maternity care, health and safety, Infection Prevention and Control, PMTCT and Family Planning through theoretical and on-the-job training, coaching and mentorship.

·         Facilitate the professional and clinical development of the individual members working within the team as well as ensuring that the resources and skills of all members of the team are used to optimal effect.

·         Utilize the full skills of a midwife as laid down in the WHO’s definition of a midwife and the Sierra Leone Nursing and Midwifery Board and provide patient care as needed at KGH and Wellbody Maternity Departments.

·         Provide clinical maternal and newborn normal and emergency obstetric care both in a mentoring and caregiving capacity, including providing cover for on call and weekend hours as needed.

·         Develop and facilitate a conducive training and learning environment for all staff thereby fulfilling the mentorship and preceptorship roles.

·         Supervise maternity staff, ensuring all patients in labor are properly monitored through the use of the partograph and that staff are confident and competent in doing so, as well as able to provide efficient antenatal, postnatal and newborn care.

·         Provide quality and timely training on basic emergency obstetric care for obstetric complications, with the requisite on the job training to ensure that all staff can carry out these activities correctly.

·         In conjunction with the RMNH Lead, Clinical Facilitator, International Midwife Mentor and OBGYN specialist, devise assessment tools to monitor staff competencies and provide practical on the job support as needed to staff who are not achieving said competencies.

·         Ensure staff are competently able to complete proper documentation in all maternal and newborn records and health and clinic registers.

·         Provide training and other technical support to Community Health Workers and Traditional Birth Attendants on RMNCH topics as needed.

·         Conduct regular monitoring visits to targeted health facilities to monitor quality and performance of mentored staff and completion of monitoring tools.

·         Work cohesively with the M & E team to collect data, compile and share activity and monthly reports with RMNH Lead, GAC Project Manager and designated others.

·         Support maternal and newborn health outreach activities in collaboration with the RMNH program manager.

·         Provide compassionate care in line with the PIH mission

·         EMONC trainer and proven knowledge of BEMONC, CEMONC, Essential Newborn Care and ETAT.

·         Experience in managing high risk pregnancies, complicated deliveries and postpartum complications.

·         6 or more years of experience as a practicing midwife in Sierra Leone or other developing country.

·         Basic proficiency with Microsoft Office (Word, Excel, PowerPoint) and Exchange Email.

·         Fluent in verbal and written English, and Krio.

·         Working with the NCD team, help manage files and data for NCD patients, ensuring files are well organized and complete, and data reporting

·         Giving appointments to new patients referred to the clinic.

·         Regularly calling patients who have missed their appointment or are lost to follow up to attend their NCD clinic

He/she does this thoroughly following the principles of inspection, palpation, percussion and auscultation where ever applicable to find out the actual cause of disease, and also utilizing the appropriate laboratory work. He/she should consult the medical officers for discussion on patient’s care plan in some critical cases, respecting patient’s privacy and confidentiality as much as possible.

The CHO will be an integral part of the NCD team at KGH. Responsibilities will include:

·         Provide NCD care during NCD home visits

·         Provide NCD care in the NCD clinic when he/she is not on home visits

·         Provide mentorship and training to nurse and other workers at the hospital.

Other responsibilities

The CHO will be asked to support other departments, which may involve taking regular calls and managing patients in other units. This may involve:

·         Responding to medical/health problems presented by patients including proper history taking, investigations, diagnosis, treatment and referral as appropriate.

·         Maintaining confidentiality and impartiality.

• Promoting continuous medical education amongst other junior clinical staff.
• Admitting patients requiring special care followed by investigations and treatment.

• Liaising with other medical and non-medical staff in the hospital to ensure quality treatment is giving to the patients, and also promotes health education.

PIH has launched a catalytic effort to radically reduce maternal mortality, creating a comprehensive solution at the epicenter of the world’s maternal health crisis. PIH will build a Maternal Center of Excellence (MCOE) in rural Kono District, Sierra Leone, which will serve as a world-class health service and training facility, providing a globally accepted gold-standard of inpatient and outpatient maternal and newborn health care, and serve as a training center where the next generation of Sierra Leonean health care leaders can develop the skills and knowledge to deliver evidence-based, high-quality care to women and children in their communities. Importantly, the success of this work will serve as a model that can be replicated for national and global scale—providing an evidence base and a blueprint for how to systematically end preventable maternal deaths around the world, beginning in the place where it is most severe. We are looking to hire an MCOE Program Associate to help support the scoping, planning, and operational work related to launching the future MCOE.

The right candidate will have a strong clinical or non-clinical background in RMNCH with technical knowledge of public health evidence on reproductive, maternal, and/or child health and an ability to lead technical writing about RMNCH interventions.

The candidate will have excellent professional communication, strong partnership cultivation skills, and an ability to steward high-level relationships with government, donor, and partner organizations to help design, plan, and effectively mobilize support for the MCOE.

The candidate will be nimble, action-oriented and flexible to changing needs of our patients and programs, willing to do what it takes and hop on new projects to deliver on behalf of those we serve. Successful candidates will have a strong ability to translate ideas into action and move projects forward.

Specific Responsibilities:

Maternal Center of Excellent Project Planning

• Project Planning & Tracking Deliverables: Collaborate with the Human Resources, Clinical, and Policy & Partnership teams to develop a clear project plan and define deliverables and timelines for all aspects of the MCOE pre-opening phase of operations
• Staffing Strategy Coordination: Coordinate the development of a staffing and recruitment strategy for the MCOE in partnership with Human Resources, Clinical Teams and Government of Sierra Leone. Conduct relevant assessment through the Directorate of Human Resources for Health to determine long-term pipeline of HR, and other relevant policy decisions related to recruitment, hiring, retention, and long-term absorption
• Clinical & Academic Training Planning: Collaborate with the Director of Clinical Programs, RMNCH Clinical Program Manager, and Director of Nursing to identify clinical training needs, coordinate with them to design program and develop operational plans for planning and implementation of new clinical initiatives
• Monitoring & Evaluation (M&E) and Research Planning for MCOE: Collaborate with M&E team and M&E consultants to define project M&E needs and support baseline assessment pre-opening with relevant M&E stakeholders and leadership
• Communication Flow: Prepare monthly updates on MCOE planning progress, key achievements, and forthcoming plans for internal and external stakeholders
• Project Coordination: Attend Management and Project team meetings, coordinate relevant activities, ensure follow-up on agreed action points across different departments
• Presentation & Documentation of MCOE Efforts: Support in presentations, proposals, or summaries for the MCOE as requested by the RMNCH Clinical Manager, Chief Medical Officer, Director of Policy & Partnership, Executive Director or other leadership
• Ad-Hoc Requests: Provide support to and perform other tasks as assigned

National Alignment, Communication, & Joint Planning

• Ensure consistent flow of information within PIH Sierra Lone through the Director of Government Relations related to the MCOE planning processes
• Identify and engage relevant Ministries into MCOE planning and steward relationships to ensure full integration of MCOE planning into national strategies, planning, and dialogues in partnership with Policy & Partnership team
• Provide updates on key MOH discussions to Director of Government Relations, Director of Policy & Partnership, Chief Medical Officer, and Executive Director
• Ensure MCOE programs are aligned with all standard government protocols, procedures, and future strategic direction, etc.

• A minimum of a Bachelor’s degree with three years of relevant work experience, with at least 2 years of experience in a maternal, child, or women’s health project in low or middle income country context
• Experience working with individuals of various skill sets and cultures, including expertise on issues of gender equity and the promotion of sexual and reproductive rights.
• Exemplary interpersonal communication skills required to interact effectively with senior leadership, partners, and clinicians
• Excellent planning and prioritization skills as well as creative problem-solving and analytical skills, including experience working and developing budgets and strong Microsoft Excel skills.
• Strong quantitative skills, including data analysis, knowledge of basic M&E principles, and ability to correctly interpret and communicate quantitative information
• Excellent ability to manage communication flow and ensure the right people have the right information at the right time
• Strong networking skills, including abilities to identify key strategic partners and cultivate relationships effectively
• Excellent communication skills verbal and written, including an ability to articulately speak about PIH’s work and vision
• Strong project management and time management skills; highly organized and detail-oriented.
• Ability to work independently and effectively in high-pressure, fast-paced environment and handle multiple priorities simultaneously
• Interest in social justice and commitment to advocating for a preferential option for the poor
